Is hyperbaric oxygen therapy needed in Hungary (too)?
Abstract:
This paper describes the concepts, brief history, treatment principles and physiology of hyperbaric oxygen (HBO) and hyperbaric oxygen therapy (HBOT). He discusses the indications and contraindications and summarises the experience gained at the Hyperbaric Oxygen Therapy Centre of the Vaszary Kolos Hospital in Esztergom since 1st November 2023. On the basis of this, the answer to the question of whether this type of treatment is necessary: clearly yes.

A guide to the basics of wound care
Abstract: When a person, as a doctor first encounters a chrinic wound, it is easy to despair. When the patient arrives, the wound is exposed, the nurse is horrified, and the scribe leaves immediately. It is true that such a wound can be repulsive and can cause discust in a layman. So it is understandable that patients hide it as long as possible and do not tell anyone about it. In such a desperate situation, they expect help from the doctor. The pressure is undoubtedly great. The simple treatment methods fail one after the other, and we ourselves do not understand the reason for our failure. The author of the article tried to compile his paper based on this experience. Its purpose is to provide guidance to colleagues who are wandering through the maze of wound care and who may be just getting to know the subject. After all, countless procedures and treatments, often contradicting each other, circulate in the public consciousness.

Possibilities of the dermatologist in the care of chronic lower extremity ulcers
Abstract: Successful management of chronic lower extremity wounds requires early diagnostics,
development and implementation of the appropriate treatment plan. Most lower extremity
ulcers develop as a complication of vascular disease, diabetes or pressure injury, while a
smaller percentage have various other etiology (atypical wounds). As the development
of a wound in a patient is often a complex process, successful treatment requires the
cooperation of multiple specialists. In the article we discuss how dermatologists, as part
of a multidisciplinary team can help ensure that patients receive correct diagnosis and
effective treatment for their chronic wounds and periwound complications.

Treatment and prevention of diabetic neuropathic foot wounds with podiatric methods
Abstract: The best wound is „the one that doesn’t even appear”. Septic, even life-threatening
wounds can develop very quickly, imperceptibly, and painlessly on the diabetic
neuropathic foot. In order to prevent them, it is important to screen the affected patient
group, assess the risks, then determine the necessary patient pathways, provide them
with adequate care, follow-up, control examinations and of course individualized
education. Based on their qualifications, podiatrists perform this task in many countries
of the world in cooperation with general practitioner practices and medical specialists
(dermatology, surgery, internal medicine, angiology, traumatology, orthopedics) that
provide care for diabetic patients. In our country only first steps have been taken on this
field. As a result, the number of diabetic amputations could be significantly reduced,
which would improve the quality of life of the patients and reduce the social burden.

Treatment of hidradenitis with radical excision, half-thickness skin grafting and vacuum therapy. Case report
Abstract: Hidradenitis is a chronic skin disease that affects the apocrine gland-bearing areas and is characterized by inflamed, painful, draining nodules, abscesses, and sinus tract formation. Most commonly it occurs in the armpits, but can also develop in the groin area, perianal region, inner thighs and breasts. The development of the disease may be influenced by factors such as smoking, genetic background, obesity, diabetes, and can have a higher prevalence in women. Treatment can be conservative (with antibiotics, biological therapy) or surgical.
During surgical interventions, the abscess cavities are opened, and the affected area is radically excised. The defect can be covered using split-thickened skin grafts or various pedicle flaps. Negative pressure wound therapy (NPWT) is a widely used procedure. The
advantages of NPWT include faster wound bed preparation, promotion of granulation tissue formation, preparation of the wound bed for graft acceptance, and facilitation of adherence of the transplanted split-thickness skin graft.
The authors present the treatment of an axillary hidradenitis that was unresponsive to conventional treatment methods, using a combination of radical excision, NPWT and
split-thickness skin grafting.

Genetic aspects of venous leg ulceration
Abstract: Venous leg ulcer is a multifactorial disease, most often caused by chronic venous disease,
but metabolic pathologies and haematological abnormalities are also important factors. Genetic factors play an important role in the development, progression and healing of chronic wounds. The aim of this review is to summarise the genetic predisposing factors (gene mutations/polymorphisms) involved in the pathophysiology of venous leg ulcers.
In rarer cases, extensive molecular genetic testing, such as whole exome sequencing, may also be of value in clarifying the aetiology of the ulcer if familial accumulation is detectable and there is evidence of coagulation, immunological or endocrine abnormalities in the medical history and laboratory findings. The clinical benefit of genetic screening in preventive conditions is that it allows appropriate treatment to be started before the development of ulcer. On the other hand, from a pharmacogenomics aspect, it may help in the future in the planning of individualised treatment and the application of targeted therapy. Overall, preventive care can reduce the burden on the health care systems and the risk of complications and even life-threatening conditions.

Reconstructive lymphatic surgery and management of chronic wound
Abstract:
Lymphedema is a chronic, debilitating, progressive disease caused by lymph flow disruption owing to mechanical and/or dyanmic causes. Initially, lymph fluid accumulation with pitting edema and lymphatic malfunction appear. Due to prolonged inflammation, adipose tissue enlargement is observed with fibrotic remodelling.
Regarding dissease progressivity early diagnosis and therapy are mandatory. The most advanced form of chronic venous disease is the active ulcer (CEAP C6). The new recent evidences were able to show the significant role of manifest lymphatic insufficiency in the maintence of venous leg ulcers therefore improvement of lymph circulation presumably has a positive impact on wound healing.
The “gold standard” treatment is complex decongestive therapy however nonresponders keep suffering from the complications of lymphedema, and disease progression leads to decreased quality of life.
In general lymphology practice, surgical treatment comprises reductive procedures such as excisions and liposuction, as well as physiological reconstructive interventions aimed at restoring of normal lymphatic flow. Physiological reconstructive interventions include supermicrosurgical lymphovenous anastomosis (LVA), lymph node venous anastomosis (LNVA), and free vascularized lymphatic tissue transplantation. The latter involves the transfer of free vascularized lymph node (VLNT), lymphatic vessel (VLVT), and lymphatic system (LYST).
Appropriate patient selection, donor site selection, and lymph vessel selection are prerequisites for an effective reconstructive lymphatic surgery.

The role of wound irrigants in modern wound care – an instructive case
Conclusion: In the treatment of complex ulcers of immunological origin, the primary priority is targeted treatment of the underlying autoimmune disease or other provoking factor. At the same time, the often used immunosuppressive therapy can increase the risk of infection due to the presence of the penetration gate represented by the ulcers. Therefore, it is important to use an antiseptic wound irrigation solution that has an appropriate antiseptic effect, but is gentle and compatible with all types of smart dressings. This can help prevent the possibility of super-infection and increase the tendency of ulcers to heal. Preparations containing polyhexanide, also known as PHMB, meet the previous criteria in all respects, and therefore can prove to be an excellent choice in the treatment of ulcers of immunological origin.
